We offer free six-week workshops to help you deal with your ongoing illness.

The Chronic Disease Self-Management Program (CDSMP) is a six-session health education program for people with any type of ongoing health condition and compliments the healthcare that an individual may already be receiving.  The purpose of the workshop is to enhance one’s skills and ability to manage their health and maintain an active and fulfilling lifestyle.  Participants learn or enhance their problem-solving and decision-making skills which enable them to confront the ever-changing challenges of living with a chronic disease.  Participants are invited to bring a family member, friend or caretaker.

Studies of class participants found that individuals that completed the workshop had better coping strategies and symptom management, increased physical activity, fewer emergency room visits and hospitalizations.  At the conclusion of the workshop, they had greater confidence in their ability to manage different aspects of their health and felt better able to communicate with their physicians.
