If you wish to complete your Intake Packet in advance of your appointment, you may download and print the Intake Packet (Part 1 and 2), complete the forms and bring them to your appointment.

The Counseling & Wellness Services at locations in Lockport and Niagara Falls provide outpatient services to Niagara County residents, children and adults, who seek help for a range of emotional and behavioral health concerns and limited physical health concerns, including depression, anxiety, suicidal feelings, psychiatric illness, substance use, relapse prevention, basic primary care and wellness services.

Hours:
Business hours are Monday 8:30 a.m. - 7:00 p.m. and Tuesday through Friday, 8:30 a.m. to 4:30 p.m.
After-hours crisis intervention is available to clinic clients by calling the Niagara County Crisis Services 24-hours-a-day / 7-days-a-week phone line at (716) 285-3515.
Regular business OPEN ACCESS WALK-IN HOURS for adult (ages 18+) new client intakes are as follows:
Lockport clinic location:
- Mondays at *1:45 pm
- Wednesdays at *11:00 am
- Thursdays at *11:00 am
Niagara Falls clinic location (walk -in appointments are currently on pause; please call the office for more information for scheduling an initial appointment):
- Tuesdays at *11:00 am
- Fridays at *11:00 am
*Please arrive at least 30 minutes in advance of the above appointment times to complete registration. If more individuals present for intake than there are appointment slots available at the above times, clinic support staff in coordination with clinic supervisors will work with individuals to be seen as soon as possible.
For children new client intakes, please call to schedule an intake appointment.
