Health Department and Department for the Aging Collaborate to Offer Free Chronic Disease Self-Manage

The Niagara County Department of Health, in collaboration with the Niagara County Department for the Aging will be offering free Chronic Disease Self-Management Workshops throughout the county.  The first workshop will be held at the Wheatfield Community Center located at 2790 Church Road, Town of Wheatfield beginning Thursday, February 6.  Another workshop will be held at the Eastern Niagara Hospital-Newfane Site, 2600 William Street, beginning Friday, February 7.  Both workshops consist of six consecutive weekly classes that run from 9 a.m. to 11:30 a.m.  

This is an evidence-based program for people with any type of ongoing health condition and compliments the health care that an individual may already be receiving.  The purpose of the workshop is to enhance one’s skills and ability to manage health, and to maintain an active and fulfilling lifestyle. Program instructors encourage caregivers to attend as well, as the program provides opportunities for caregivers to learn, share and support each other.

If you have a chronic health condition such as heart disease, arthritis, diabetes or chronic pain, or if you are a caregiver for someone with a chronic condition, please contact please contact NY Connects through the Office for the Aging at (716) 438-3030 or the Niagara County Department of Health Nursing Division at (716) 278-1900 to register for the class.  The Health Department hours are Monday through Friday, 8:00 a.m. and 4:00 p.m.
