Programs determined within this unit are:

  • Assisted Living
  • Hospice Residential
  • Intermediate Care Facility (ICF)
  • Chronic Care Medicaid in a Hospital
  • Permanent Nursing Home Placement
  • Short Term Rehab 29-days or less
  • Temporary Stay - up to 6-months for rehabilitation
In order for a determination to be completed, all programs require filing an application and submitting program specific verification.

If you are in need of services, please call our office at 716-439-7728 and request a Medicaid application packet be sent to you.  Once you complete the application and gather the required documentation listed in the packet, you can either mail the contents or bring them into the office listed above.

When our office receives the application (remember to signed and dated on the back page) and verification, we will review it.  At which time, you may be sent a list of remaining verification requirements to determine financial eligibility.  In order to continue with the review, the additional verification requirements will need to be sent back to us by the given due date.

The date the valid application is received by the agency, is the date of your application.  If you are in need of coverage for medical bills prior to this date, you must request this and provide documentation for the outstanding medical expenses.

The resource limit for a single applicant is $14,850. and for a couple, it is $74,820.

To determine if you have transferred or gifted, a five-year (60 months), look-back review will be done of your resources starting with the date of your application.  Penalties are assessed on all resources that are transferred or gifted.  The penalty is $9,442 per month beginning with the month you are determined eligible to receive Medicaid.
